  4. Agree to complete a CGM start orientation offered via web-based learning, virtual or in-person instruction by a CGM-trained vendor representative.
  5. Agree to advise the NBIPP office if they discontinue use of their sensor technology.
Do I need a prescription to qualify for coverage?
Yes, you will need a valid prescription for Dexcom G6 or G7 as part of the coverage approval process.
If you are switching from Dexcom G6 to Dexcom G7, you should obtain a new prescription for Dexcom G7 from your prescribing healthcare professional.
What do I need to do to get coverage approval?
Your healthcare provider who supports your diabetes management must complete the CGM Medical Eligibility Approval form on your behalf and submit it directly to the NBIPP business office at [email protected]. This can be a diabetes specialist, General Practitioner, Nurse Practitioner, or Certified Diabetes Educator.
You must also complete and submit the Application for CGM Coverage form along with supporting documentation, directly to the NBIPP business office at [email protected]. This form collects information on your health insurance coverage (if applicable), household size and income that are used to determine your eligibility for coverage and contribution amount. It is recommended that you confirm the details of any private health insurance you have before applying for coverage.
NEW. An Online Client Application Tool is now also available for Continuous Glucose Monitoring Sensor Coverage and must be used for all new applications after September 1st, 2024. Find it HERE.
How long am I approved for?
All applicants must reapply each year.
Where can I get my Dexcom CGM supplies?
If approved for coverage, the NBIPP will notify Dexcom Canada, and we will contact you to coordinate your order and shipment of your CGM supplies. Dexcom receivers may also be available at no cost for those who do not have a compatible smart device.
Will I have any out-of-pocket cost?
The NBIPP is the payor of last resort, after private or third-party insurance coverage is used. You may be required to pay a contribution amount (copay) as determined by the Program towards the cost. Between your private insurance and the Program, you should have minimal out-of-pocket expense.
