There are three steps to get a Dexcom Continuous Glucose Monitor (CGM) and we can help you navigate the process. If you need help at any time, please give us a call toll free at 1-888-738-3646.

 


Patient Information

This necessary information initiates the process.


Insurance Information

Most insurance companies cover CGM for people with Type 1 diabetes.


Doctor Information

CGM systems require a prescription from your doctor. Some insurance companies also require information from your doctor.

What You Need to Do

Complete and return the patient information form.

Complete and return the patient insurance information form. (This gives Dexcom the information and permission to talk with your insurance company.)

As long as you complete and return the patient and insurance information forms, you do not have to do anything.

What Dexcom Will Do

We will use this information to help navigate paperwork with your doctor and insurer. After this is complete, we will contact you to collect any co-pays necessary and confirm shipping information.

We will work with your insurance company to ensure they have all of the information they need. If the Insurance company requires additional information from you (e.g. blood glucose logs), we will notify you.

We will compile the necessary paperwork and send it to your doctor. For most people, the basis of our doctor paperwork is the "Certificate of Medical Necessity" (CMN).1

What Your Doctor & Insurance Company Will Do

 

Your insurance company will review the information provided and make a coverage decision.

Your doctor is required to complete and return the paperwork we send them. Your doctor will often coordinate training in their office and/or a local diabetes center for your new Dexcom CGM.
